What the inspector wrote

On 06/04/2025, Licensing Program Analyst (LPA) Valerie Mireles met with Director Sonja Stephens for an unannounced case management inspection. A complete file review was conducted prior to today's inspection. LPA toured the facility inside, outside and a census was taken. The purpose of today's inspection was to address an unusual incident that took place at the facility on 05/21/2025. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office regarding an incident that occurred on 05/21/2025 involving a daycare child that fell while running and tripped on the plastic barrier that divides the planter box from the rest of the playground. The child was running from the woodchip planter box towards the cement and lost balance over the plastic barrier landing onto the cement. The child sustained an injury to the left arm. On 06/04/2025, LPA spoke with Director Stephens and Staff #1, who reportedly witnessed the incident as it occurred and was in proximity of the child when the incident occurred. It was reported that the child ran past Staff #1, who was standing inside the planter box, towards the cement. The child tripped, fell and landed on the child's left arm. The Director rendered aid to the child by applying an ice pack, contacted the child\342\200\231s parent and the child was picked up shortly after. The child was taken to Urgent Care that same day and it was determined that the child sustained a fracture to the left arm. A cast was applied to the left arm and the child was excused from daycare for two weeks. During today's inspection, LPA Mireles observed the outdoor planter area where the incident took place. The outdoor planter was inspected, was in good condition with no visible defects and had several inches of wood chips inside the planter box. At the time of the incident there were four teachers present and approximately 48 children at the outdoor playground area when the incident occurred; therefore, adequate supervision was in place. Director Stephens placed a work order for additional wood chips to place inside the plastic barrier and is looking into padding to place on the cement side of the barrier to absorb falls. Continued to LIC809-C. Based on the information obtained, LPA determined staff handled the incident correctly and reporting requirements were met. After interviewing staff and reviewing facility records, LPA determined facility staff took appropriate measures to address the child\342\200\231s injury, following proper policies and procedures and no regulations were violated. Per the California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today\342\200\231s inspection. Exit interview conducted with the Director, Sonja Stephens.
