What Happened
Child #1 found unsupervised outside classroom for 1-2 minutes following parent drop-off on 05/07/2025. Child followed parent outside and remained unattended in fenced area. Type A violation cited for inadequate supervision. Facility has implemented corrective measures including updated drop-off/pick-up procedures and installation of additional interior classroom gate.
