What the inspector wrote

On 3/11/2025, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced Case Management-Incident visit. During today\342\200\231s visit, LPA met with Site Supervisor, Michelle Higginson. The reason for the Case Management visit was to follow up on an incident that occurred on 2/26/2025 and the LIC 624 form that was submitted due to that incident. The LIC 624 form documented that on 2/26/2025 a child was climbing on a play structure, slipped, and hit his mouth on a step making his front teeth loose. Mother picked up child, took him to the dentist. Dentist stated that the child\342\200\231s teeth needed to be removed but could not do it at the time due to the damage and bruising of the mouth and gums. Child returned to care the next day and has been continuously attending. During today\342\200\231s Case Management visit LPA interviewed Center staff and inspected the playground and play structured where the child fell and hit his mouth. Through interviews it was determined that the child had supervision, play structure was in good condition, and that this incident was an isolated one. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiency has been cited during today\342\200\231s visit. Site Supervisor, Michelle Higginson was provided a copy of appeal rights. A notice of site visit (LIC 9213) was given and must remain posted for 30 days. This report shall be made available to the public upon request. Exit interview conducted and report was reviewed with Site Supervisor, Michelle Higginson.
