What the inspector wrote

LPA interviewed Site Supervisor and several staff members. LPA reviewed facility files. Based on LPA's observations and interviews, it was determined that the facility did not follow safe sleep practices by allowing a sleeping infant, Child #1, to be left alone in the sleeping room with no direct supervision. Therefore, the preponderance of evidence standard has been met, and the allegation is found to be Substantiated. Per California Code of Regulations, Title 22, Division 12, Chapter 3, the following deficiency is found: (See LIC 9099-D). An Immediate Civil Penalty was assessed in the amount of $500. Exit interview conducted with Vanessa Crane.
