On June 17, 2021, Licensing Program Analysts (LPAs) Brannon and Moctezuma met with Site Supervisor, Tammy Yeager. During today's visit, LPAs took a census, took outside measurements, toured facility inside and outside. Licensing was informed of new pastor Patrick Davis. The new pastor is fingerprinted cleared, However, licensee did not submit required documentation for the new pastor, who is now in charge of the Board, and makes the final decisions for the preschool. Licensee will provide copy of outside facility sketch. Required documentation: LIC 309 - Administrative Organization; LIC 508 - Criminal Record Statement, Immunization (MMR, Pertussis and Flu vaccine or declination statement for the flu vaccine), LIC 9108 - Acknowledge of Reporting Child Abuse, General Child Abuse Training and Child Care Provider Child Abuse Training that meets AB1207, Record Keeping Orientation, LIC 503 - Health Screening and TB test results that are not more than 1 year old. LPA and Title, discussed the Community Care Licensing (CCL) website www.ccld.ca.gov which will provide access to the Provider Information Notices (PINs), Quarterly Updates, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit. Exit interview conducted with site supervisor, Tammy Yeager. A copy of this report needs to be placed in facility file for public review. A Notice of Site Visit was posted on parent board for 30 days.
