What Happened
LPA conducted a case management inspection to address several incidents that had been reported late to Community Care Licensing. Based on interviews and the director's admission, the facility was cited for failing to report incidents involving personal rights violations and a child left unattended within the required timeframe.
Observations
- Visit addressed multiple incidents from March and April 2024 involving personal rights concerns, a child left alone, and mishandling of a child
- Inspector determined the facility failed to report the incidents within the required time frame
- Deficiencies were cited for violation of reporting requirements

(d) Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.(1) Events reported shall include the following:(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Director reported multiple incidents that occurred in March 20, 2024, April 11th, 14th, and 18th, 2024 on April 24, 2024. Director failed to report the incidents to the Department by telephone within the Department’s next workday of the occurrence of the incident.This requirement was not met as evidenced by Director and Site Supervisor self admitting they failed to report the incidents within the allotted time frame. This poses a potential risk to the health, safety, or personal rights of children in care.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: interviews and self-admission of director and staff. The lack of supervision made it possible for child #1 to be left alone outside for approximately 3 minutes. This poses an immediate risk to the health, safety and or personal rights of children in care.
