What Happened
Type A violation: Child left unattended outside during transition, wandered into empty 4-year-old classroom and remained unsupervised for approximately 10 minutes. Facility failed to provide adequate supervision.

(a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule,coercion, threat, mental abuse or other actions of a punitive nature including but not limited to…This requirement was not met as evidenced by: Based on staff interviews and staff admission, staff members violated the personal rights of those in care. This poses a potential risk to the health, safety, or personal rights of children in care.
Responsibility for Providing Care and Supervision. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: information obtained via an unusual incident report and self-admission of director. The lack of supervision made it possible for child #1 to be left alone in a classroom for approximately 10 minutes. This poses an immediate risk to the health, safety and or personal rights of children in care.
