What Happened
Incident investigation following Unusual Incident Report filed for event on 8/5/2024. Staff member forcefully held child down on cot and hit child on head multiple times. Type A citation issued for immediate risk to health and safety. Witness statement and child account confirmed incident.

- Staff member forcefully held child down on cot during nap time
- Staff member forcefully hit child on head multiple times
- Child reports being pushed to bed and hit on head, causing pain
