An exit interview was conducted with Administrator, Melissa Pershall. A copy of this report and Appeal Rights were provided and discussed with Ms. Pershall. A Notice of Site Visit is to be posted for 30 days.

    Responsibility for Providing Care and Supervision. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement is not met as evidence by: Administrator admiting that staff were not monotring children at all times, resulting in a child sratching another child, and staff not having knowledge of the incident until the following day.
