OPERATION OF A FAMILY CHILD CARE HOME
During inspection, the cupboard doors located under the sink were not locked. When LPA opened the doors to inspect the area, LPA observed there to be Clorox bleach. The cupboard doors did have a child lock on one of the handles, however it was not locked, therefore accessible to the children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

During record review and observation, the fire drill log that Licensee provided appeared to be altered. It appears that Licensee wrote a 4 over the 3 in 2023 to indicate 2024 for fire drill, smoke alarm drill, and carbon monoxide drill. The same thing appears for the year 2022, Licensee wrote a 3 over the 2 in 2022 to indicate 2023. There is also no documented time of day that the fire drill was conducted. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews and record reviews, Licensee did not have immunization records for five out of the fifteen children that are enrolled in the facility. The Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
