Inspector’s narrative

What the inspector wrote

On 2/6/26 at 10:15am, an annual inspection was made to the facility by Licensing Program Analyst (LPA) Nicolette Cunningham, who met with licensee Dena Bott. The facility license is currently under probationary status. At 10:30am the home was toured inside and outside. The licensee and two assistants were supervising one infant and eight children, and operating within the licensed capacity and ratio requirements. The facility’s operating hours are 5:00am-6:00pm, Monday–Friday. The floor plan submitted by the licensee was reviewed and verified. The off-limits areas of the home are all bedrooms, and were made inaccessible by door locks or door knob covers. The children use the back yard as the outdoor play area and it is fully fenced. There were no pools or other bodies of water observed in the yard. There is currently one adult living in the home. Six children's records were reviewed at 11:55am. The licensee’s record and two staff files were reviewed at 12:20pm. Records were complete. The following deficiencies were cited: home is not clean and orderly, broken toys and fixtures, items causing unsafe environment, and personal rights for uncomfortable environment (see LIC 809D). Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Cunningham informed licensee D. Bott that this report dated 2/6/26 document(s) four Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Cunningham informed the licensee to provide a copy of this licensing report dated 2/6/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Licensee Bott was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Bott, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.     Based on observation, the licensee did not comply with the section cited above due to large cobwebs throughout the home, dust covered furnace filter, fan over lunch table had a thick cover of dust, popcorn ceiling debris was on the fireplace, debris on floors, trash can had food particles on it, carpets contained dirt and stains, inside of refrigerator contained food particles and home had foul odor which poses an immediate health, safety or personal rights risk to persons in care.

    Based on observation and interview, the licensee did not comply with the section cited above in multiple broken toys, broken glider chair, and broken fixtures throughout the home which poses an immediate health, safety or personal rights risk to persons in care.

    Based on observation and interview, the licensee did not comply with the section cited above in multiple large pieces of furniture were not secured, several items were stacked up high and a power strip was plugged in on the front porch with outlets uncovered which poses an immediate health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above due to the home had a foul odor, the furnace filter was extremely dusty, the fan above table where children eat had a layer of dust and small toys, papers, stickers, toothpick and debris were throughout the home which poses an immediate health, safety or personal rights risk to persons in care.
