On this date 03/10/2025, an unannounced Case Management - Other Inspection was conducted by Licensing Program Analysts (LPAs) Sonja Navarrette and Lady Cabrera. LPAs met with Tiffeny Nunes to review the below missing documents for the director's packet that was submitted to the Department. Missing documents: Preventive Health & Safety Certificate EMSA certified CPR/First Aid Certificate Health Screening (only blank form submitted) TB test results Immunizations for pertussis, measles, and influenza LIC500-Signature on second page LIC610-Emergency disaster plan LIC9108 AB1207 Mandated Reporter training certificate Operations and Record Keeping Orientation Certificate. Exit interview conducted and report was reviewed with Designated Representative Tiffeny Nunes. Appeal rights were provided. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS.
