What the inspector wrote

On 05/28/2025 Licensing Program Analyst (LPA) Sonja Navarrette conducted an unannounced Plan of Correction (POC) inspection. LPA with Director Tiffeny Nunes to review the POC’s associated to the deficiencies cited on 05/21/2025. Today LPA verified the following : · Director submitted a written plan of procedure for children’s sign in and sign out sheets on 05/27/2025. · Director has placed a notification sign at sign in and sign out desk to alert parents of new procedure. · Director has provided each parent with a copy of the new and updated sign in and sign out policy. · Facility has purchased patricians to make adult room and main hall inaccessible and use side exits. · Director is implementing regular checks to ensure that physical barriers remain intact and effective. · Director posted “No entry during business hours” outside of side exit, inside and outside of adult room. Access from inside adult room has been locked. LPA cleared the deficiencies on this date and provided licensee a “Letter of Deficiency Citations Cleared.” This letter must be filed with the facility for 3 years and upon request made accessible to the public for review. Exit interview conducted and report was reviewed with Director Tiffeny Nunes and was provided with appeal rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS
