What Happened
This was a case management visit for an incident in which a child was left alone in the outdoor play area for less than two minutes. Staff said the child had hidden behind a pillar during the transition back inside, and the incident was reviewed with the site supervisor.
Observations
- Three staff were supervising 16 children in the outside play area when the incident occurred.
- The child was left alone outside for less than two minutes after the group came back inside.
- Staff notified the parent and the Fresno Regional Office by telephone.
