What Happened
No children were present because the visit occurred during a staff training day, but the record review still found incomplete staff documentation. The main issues were missing pertussis immunization proof and missing mandated reporter training for staff.

Based on observation and interview, the licensee did not comply with the section cited above. During naptime, LPAs observed one child who laid awake while the other children were napping. Staff members were engaged in a meeting in the back of the room leaving the children without visual supervision at all times which poses a potential health, safety or personal rights risk to persons in care.
