What the inspector wrote

On 05/28/2024, Licensing Program Analyst (LPA) Meche Rosales conducted a Case Management Incident inspection for a phone call/ Unusual Incident Report (UIR) that was received at the Fresno Regional office. LPA met with Licensee, Rigoberto Ramirez Jr. Other licensee Maricela Ramirez (wife) was at an appointment. Rigoberto stated they are closed for the day due to his wife’s appointment. LPA toured facility inside and outside and took census. On 05/15/2024 a telephone call was received at Fresno Regional office from the licensee Maricela reporting an incident that occurred with one of the day care children. Licensee Rigoberto called wife Maricela to go over the incident with LPA as Rigoberto was upstairs at the time of the incident. Upon interview with licensee Maricela over the phone and Rigoberto in person, both licensees stated that the incident happened so fast. On the day of the incident that occurred on 05/14/2024 around 5:15pm, there were seven children present, and they were all being picked up because it was the end of the day. Licensee stated that she usually places that child in the playpen while she is cooking because the child likes to crawl/walk around the cooking area and she observed child was crying in playpen while dinner was being made, but Licensee thought child was hungry. Licensee finished making food and got children served, Licensee observed child was not eating as much and when she touched child's head, she felt warm. Licensee stated she began to hold child and noticed child's eye going to the side. Licensee started calling child's name and observed child did not respond, was drooling and started shaking her legs. Licensee observed child's lips turning purple and mom showed up as the incident occurred. 911 was called and EMS showed up to assess the child. According to the ring camera 911 was called and EMS arrived at 5:25pm on 05/14/2024. Child was taken to the hospital and released at night. The licensee stated that the childs mother said that this type of incident had occurred to the child’s older sibling but not to the present child. Parent stated doctors confirmed when the child runs a higher-than-normal fever that is what caused the child to have a febrile seizure. According to the licensees, the child was cleared to return to the day care the following day however the child did not return until 05/17/2024. Licensee stated moving forward the protocol with this child she has implemented continuously monitors child’s temperature and she keeps in constant contact with child’s mother. Exit interview was conducted with Licensee, Rigoberto Ramirez Jr. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
