What Happened
Found deficiencies in hazardous item storage accessibility, inadequate supervision resulting in infant climbing to counter twice, missing nap log documentation, and outdoor play area hazards including step ladder and protruding bolts.
Observations
- Off-limit rooms were accessible - chain locks on bedrooms and garage not secured
- Laundry detergent accessible in garage
- Cleaning products (Windex) in accessible bathroom sink cabinet with unsecured latch
- Infant child #4 (under 18 months) climbed onto kitchen counter without licensee noticing
- Approximately 60 minutes later, child #4 again climbed onto bar stool creating fall risk
- Step ladder approximately 4 feet in height present in outdoor play area
- Several bolts protruding approximately 3/4 inch at bottom of multiple posts in outdoor area
- Licensee did not have sleep log for infant in care

Based on observation and interview, the licensee did not comply with the section cited above. Today, LPA Ocegueda observed multiple cleaning supplies (Windex, disinfectant wipes) that read "keep out of reach of children" under the accessible bathroom sink. Today, the garage was accessible and there were multiple accessible detergents at arm’s reach of young children. Outside, LPA observed a 4-foot latter and multiple protruding bolts on wooden posts. This poses a potential health, safety or personal rights risk to persons in care.
What does this citation mean? →Based on observation the licensee did not comply with the section cited above. Today, LPA observed an infant (child #4) climb onto a bar stool twice and onto the kitchen counter once. Although the licensee and assistant #2 were in close proximity, adequate supervision was not being provided as child was able to climb the stool and onto the counter. Licensee confirmed the observation. This poses a potential health, safety or personal rights risk to persons in care.
