What the inspector wrote

On 3/13/2024, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Courtnee Hoogland. The purpose of this inspection is in regards to a self reported incident telephoned by Director Hoogland to the Officer of the day on 2/29/2024. On 2/28/2024, eleven children from the Blue classroom had just finished using the restroom and were in line to go outside to the playground. Teacher A & B were supervising the eleven children. Child #1 left the line and returned to the restroom. Neither Teacher A nor Teacher B saw child #1 leave the line and the 2 teachers along with 10 children proceeded to the playground. Teacher C observed child #1 through a window in the restroom. Teacher C checked on child #1 and realized he was alone. Teacher C notified Director Hoogland. During pick up, staff notified the father of child #1 and on 2/29/2024, a follow up phone call was made to notify child #1 mother. Director Hoogland also submitted the Unusual Incident report LIC624 to the licensing office on 2/29/2024. Director Hoogland explained this incident was the result of confusion from the facility sign in/sign out log. On 2/28/2024, a parent signed her own name for a child's name on the wrong line which caused confusion for afternoon teachers as a child being signed out. LPA Marquez determined that although adequate ratios were in place when the incident occurred and Director complied with timely reporting requirements, a lack of proper supervision resulted in the child being left unattended and staff failed to review the number of children signed in to the actual number of children present. Continued on LIC809-C Per California Code of Regulations Title 22 Division 12 Chapter 1, the following deficiencies are being cited today (See page LIC9099-D). LPA Marquez informed Director Hoogland that this report dated 3/14/2024 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Marquez informed Director Hoogland to provide a copy of this licensing report dated 3/13/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and report was reviewed with Courtnee Hoogland. A copy of Appeal Rights and the Notice of Site Visit was given and must remain posted for 30 days.

  • SIGN IN AND SIGN OUT

    SIGN IN & OUT -In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, & implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following:The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. This requirement was not met as evidenced by an Unusual incident report submitted to the licensing office on 2/29/2024. This poses a potential risk to the health, safety, or personal rights to children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    CARE AND SUPERVISION- The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not met, as evidenced by Director's self reporting phone call to licensing office (see LIC809 for further details). This poses an immediate risk to the health, safety, or personal rights of children in care.
