On 09/09/2025, Licensing Program Analyst (LPA), Sonja Navarrette conducted an unannounced Case Management inspection. The purpose of the inspection was to follow up on an unusual incident report (UIR) that was reported to the Department on 08/20/2025 regarding children personal rights. Today, LPA Sonja Navarrette met with Site Supervisor Tasha Pesic, toured the facility and took a census of the children. LPA was notified upon arrival that the assigned Site Supervisor Cynthia Ruby Alvarado was not present at facility today. LPA was informed that some of the parties involved were interviewed by Human Resources. LPA obtained information during the interviews and observed video footage. Additional interviews will need to be conducted to determine if facility ensured child\342\200\231s needs were being met. Per California Code of Regulations, Title 22, Division 12, no deficiency cited. Exit interview conducted and report was reviewed with Site Supervisor Tasha Pesik. A notice of site visit was given and must remain posted for 30 days.
