What the inspector wrote

On 09/18/2025, Licensing Program Analyst (LPA), Sonja Navarrette conducted a Case Management inspection. The purpose of the inspection was to follow-up on an unusual incident report that was reported to the Department on 08/15/2025. Incident was regarding Child #1 (C1) fingers were allegedly stepped on by Staff #1 (S1). C1 was heard saying \342\200\234Ow\342\200\235 and asking S1 to remove their foot from C1\342\200\231s hand. S1 did not remove their foot and repeatedly said, \342\200\234Oh this one\342\200\235 until their foot was off C1\342\200\231s hand. Today, LPA Sonja Navarrette met with Director Cynthia Ruby Alvarado, toured the facility and took a census. In regard to the incident, LPA interviewed staff and reviewed video footage. Inconsistencies with interviews conducted and documents reviewed, incident appeared to be isolated. S1 denied stepping on C1\342\200\231s fingers. LPA were informed that Director had followed the facilities\342\200\231 procedures to ensure the child\342\200\231s needs were being met. This appears to be an isolated incident, and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency is cited during today's visit. Exit interview conducted and report was reviewed with Director Cynthia Ruby Alvarado. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
