What the inspector wrote

On March 3, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management\342\200\223incident inspection. LPA met with Site Supervisor Claudia Ramos and a census was taken. LPA toured the inside and outside of the facility. The purpose of today\342\200\231s inspection was to follow up on the unusual incident report received by the Fresno Regional Office on 01/23/2026. On 01/22/2026, Child #1 (C1) was running to get water from the sink inside the classroom when he tripped and hit his forehead on the edge of the sink. Staff #1 (S1) and Staff #2 (S2) observed the incident and observed that C1 had an abrasion on his forehead. C1\342\200\231s mother was contacted, and C1 was taken to the hospital for evaluation. Staff was later informed that C1 required stitches. During today\342\200\231s inspection, LPA conducted observations, records review, and staff interviews. Based on the interviews and observations, LPA determined that there was no lack of supervision, staff took appropriate action, and there were no hazards inside the classroom that contributed to C1 tripping. LPA recommended Technical Support Program to Site Supervisor regarding active-supervision training and Site Supervisor stated she will also request additional training from the school district. An exit interview was conducted, and the report was reviewed with Site Supervisor Claudia Ramos. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
