Pre-licensing visit·September 23, 2025

On 09/23/2025, Licensing Program Analysts (LPAs), Stephanie Vega-Gonzalez and Nancy Her conducted an announced Pre-Licensing inspection for a Single License Conversion. This facility was previously licensed under 540403064 and 543810202. Upon arrival, LPAs met with Facility Representative Kim Sa. Facility Representative is requesting to be licensed for 100 preschoolers ages 2 to 6 years in classrooms 102, 103, 104, 105; 16 toddlers ages 18 months to 36 months in classroom 106; and 16 infants ages 4 weeks to 24 months in classroom 107. Hours of operation will be Monday through Friday from 7:00am to 5:30pm. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed the Facility Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, the Facility Representative stated that they will be in the front office. There is no kitchen on site, children bring their own food from home, including infants. There is an operational carbon monoxide detector on site located in the classrooms. All required licensing documents were observed posted in the front office. Children will be signed-in in the front of the office and signed out in their designated classrooms. LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for preschool component is 2,611 square feet, which is insufficient to accommodate the overall capacity of 100 Children. There is only enough square footage for 74 preschool children. Total indoor activity space measured for the toddler component is 491 square feet, which is sufficient to accommodate a capacity of 14 toddler children. Total indoor activity space measured for infant component is 316 square feet, which is sufficient to accommodate a capacity of 8 infant children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, cribs, bookshelves, and other activity supplies for the children. Infant toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via water fountains and personal water bottles. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 09/05/2025 by City of Tulare Fire Prevention. In the infant rooms there are feeding chairs and feeding tables that have broad-based legs, plastic seats are in good repair and trays lock onto chairs. The feeding chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high. Diapering sink is not used for meal preparation or dishwashing. There are 4 cribs that meet regulatory requirements. The crib area is separated from activity space via an enclosed partition wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the infant and toddler classrooms. Needs and Services Plans and Sleep Plans are kept in the front office. LPAs observed a total of 8 sinks , 8 toilets, and 1 urinal available for preschool children’s use. LPAs observed a total of 1 sink and 1 toilet available for toddler children’s use. The infant room currently does not have at least one potty chair for infants, the Facility Representative will be obtaining one. These are sufficient to accommodate an overall capacity of 74 preschool children, 14 toddler children, and 8 infant children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the front office. The facility currently does not have a fully fenced playground area for each component. Facility is requesting to share the infant outdoor area with the toddler component. A waiver will need to be submitted and approved prior to licensure. Fencing for the infant yard is at least four feet high. The preschool outdoor area is fully enclosed by building walls and fencing that is at least four feet high. The total square footage for all the outdoor activity space for the preschool is 5,568 square feet, which is sufficient to accommodate the overall capacity. The total square footage for all the outdoor activity space for the infants is 1,705 square feet, which is sufficient to accommodate the overall capacity. Shade is provided via canopies. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. There is a climbing structure on the preschool and infant playground for children, and it is properly anchored. LPAs did not observe a manufacturer’s age label on the climbers and reviewed findings with Facility Representative. There is adequate cushioning in fall zones of climbers provided by rubber pour. Drinking water is available via water fountains and children’s personal water bottles. LPAs observed in the preschool outdoor area that the wooden steps for the monkey bars would need to be repaired. LPAs also observed that the rubber on the playground climber needed to be repaired. Facility Representative was informed that there are blind spots along the outdoor hallways that have iron fence doors. The Facility Representative stated that children will be fully supervised and that the facility will submit a plan to CCLD on how staff will ensure areas will be fully supervised. The Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). The Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPAs discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPAs also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The Facility Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed prior to the issuance of the license: · An updated LIC 200A and updated board of resolution regarding the new capacity for each component. · A waiver request needs to be submitted to the Department for the two year olds from the preschool component and the toddler component to share the infant outdoor area. · A waiver to utilize the church restrooms in front of the preschool playground outdoor area for the preschool component needs to be submitted to the Department. · The infant and toddler classrooms need their diaper changing tables to be within arm’s reach of a sink. · In the toddler classroom the cabinet needs to be anchored. · In the toddler classroom there needs to be 2 additional sleeping mats. · The preschool and infant playground climbers needs to be evaluated for an appropriate age label. · The preschool playgrounds wooden steps for the monkey bars need to be repaired.
