On March 3, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management–incident inspection. LPA met with Director Kim Sa, and a census was taken. LPA toured the inside and outside of the facility. The purpose of today’s inspection was to follow up on the unusual incident report received by the Fresno Regional Office on 01/12/2026. On 01/12/2026, Child #1 (C1) had their pinky finger on the left hand smashed by the restroom door. Staff #1 (S1) closed the restroom door while C1’s finger was at the latch jamb, which caused a crush injury, nailbed laceration, and an open fracture of the phalanx of the left pinky finger. S1 immediately contacted the Director regarding the incident. The Director contacted the mother, and C1’s mother and father took C1 to the hospital. C1 was admitted to the hospital at 4:45 p.m. and discharged at 11:31 p.m. During today’s inspection, LPA conducted observations, records review, and staff interviews. Based on the interviews and observations, LPA determined there was no lack of supervision or personal rights violation. Staff took appropriate action, and there were no hazards inside the classroom that contributed to C1’s injury. An exit interview was conducted, and the report was reviewed with Director Kim Sa. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
