On April 20, 2026, Licensing Program Analyst (LPA) Xona Xayavong and Licensing Program Manager (LPM) Cynthia Brannon conducted a case management – other inspection. LPA and LPM met with Director Kimberly Sa. LPA and LPM toured the facility inside and outside, and a census was taken. During today’s inspection, LPA and LPM observed the following: In the main playground, there were multiple tears in the pour in place rubber flooring; cracked and peeling exterior walls outside the classroom; tears on the corners of the north and south shade covers; and blind spots inside the tunnel of the play structure and the rock-climbing play structure. Per director, staff follows a protocol when children are playing in these areas. In the front playground between classrooms 102 and 103, LPA and LPM observed two tricycles with missing handles, two small trikes with dirt and rusted bolts, cracks in the plastic play structure, and gaps in the pour in place rubber flooring. Per director, licensee, First Baptist Church Tulare are gathering bids for the rubber pour cushioning. Director removed play equipment in need of repairs or replacement. LPA and LPM observed inside classroom 102 that a child from classroom 103 walked into the bathroom without visual supervision. The door to classroom 103 was partially closed, and the two staff members in classroom 102 did not notice that the child had entered the bathroom. LPM informed staff and was addressed with director. LPA and LPM also observed in classroom 104 that the cots were stacked with sheets touching each other. The Director stated she will ensure cot covers are tightened to prevent cross-contamination. (Continue on LIC 809-C) In classroom 105, LPA and LPM observed a bookshelf positioned in the middle of the classroom, which obstructed visual supervision when children were kneeling down or when staff were seated. LPA and LPM observed two staff seated and unable to see the children from their sitting positions. LPM spoke with director. The classroom has another table for activities across from the bookshelf. Director reviewed with staff. LPA and LPM also observed in classroom 107 that the sink and changing table were not within arm’s length. Staff moved the changing table to be within arm’s length of the sink during today's inspection. Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiencies were cited. An exit interview was conducted with Director Kimberly Sa. The Director was provided with a copy of this report and appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
