What Happened
One Type A violation: facility failed to comply with criminal record clearance regulations (two uncleared adults), failed to report incident timely, and used prohibited second-floor area for childcare despite fire safety violation.
Observations
- Uncleared Adult 1 provided care and supervision of children for at least one to two weeks
- Uncleared Adult 2 attended facility more than twice to observe daycare routine
- Infant taken upstairs to Room 2 for nap (designated off-limits per Fire Safety Inspection 01/18/2024)
- Second floor deemed non-compliant with fire exiting requirements
- Incident not reported to CCL until 11/01/2024 (delayed from 10/18/2024 incident date)
