What the inspector wrote

On 03/02/2026, at 9:15 AM, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Annual Random inspection of the above-mentioned Child Care Center (CCC). LPA met with Julie Shannon, Head of School and explained the purpose of the inspection. The CCC is located on the grounds of Bethany Christian School and operates Monday through Friday 7:30 AM \342\200\223 5:15 PM. The facility uses four classrooms for child care services: Room 101 (3 years of age), Rooms 102 and 103 (Pre-Kindergarten age), and Room 209 (2.5 to 3 years of age on Tuesday\342\200\231s from 9:00 AM to 11:00 AM). Rooms 101, 102 and 103 are located on the first floor of the CCC, while Room 209 is on the second floor of the CCC. At the time of inspection, LPA observed thirty-six (36) children on site, along with six (6) staff (cleared and associated) providing care and supervision. LPA was informed that the Director was absent due to illness. The CCC is clean and orderly. LPA observed required licensing forms and documents posted on a bulletin board on the main walkway to the CCC. LPA observed the CCC's Sign In/Out forms. The forms contained the arrival and departure times of children in care as well as signatures from the children's authorized representatives. Each classroom of the CCC has age-appropriate equipment and furnishings available for children in care. LPA also observed waste receptacles throughout the CCC with tight fitting lids. The CCC has a carbon monoxide detector in the hallway between Room 101 and Room 102 which was tested at 9:48 AM and found to be operable. LPA observed the restrooms to be in safe and sanitary operating condition. The CCC has a refrigerator in the Director's office which is capable of storing food and beverages for children in care. Continued on 809-C Lunches and snacks are provided by the parents of children in care. Children can also receive (purchase) lunches from a school vender. The CCC has available drinking water for children inside and outside of the facility. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives s ection 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) Medication at the CCC is stored in a centralized location (Main Office) which is inaccessible to children in care. Sharps and cleaning compounds in each classroom are in locations which are accessible to staff members, but inaccessible to children in care. Each classroom has a Fist Aid K it. The CCC\342\200\231s outdoor playground area is enclosed by a combination of chained link and wrought iron fencing. The fence\342\200\231s entry/exit gates are secured. The surfaces of the outdoor playground areas are varied. LPA observed a locked metal cabinet which contains CCC's supplies. LPA observed age-appropriate play equipment in the play area. The play equipment is free of sharp, loose or pointed objects. The play structures in the playground area have adequate cushioning materials to absorb falls and shade created by building overhangs and shade sails. LPA observed no bodies of water at the CCC. LPA reviewed children, staff, and facility records. The children\342\200\231s records sampled were complete and found to contain emergency contact information, immunization records, and other required licensing forms. Staff records were reviewed and found to be current with CPR/First Aid training, qualifications, and other required licensing forms. Continued on 809-C LPA notes the last disaster drill at the facility occurred on 02/03/2026. LPA notes there is a firearm and ammunition on site as the CCC employs an armed security guard. LPA verified the CCC has a waiver for regulation 101238(g)(2). This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility representative was reminded that all adults 18 and over , including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
