What the inspector wrote

On 4/08/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of completing a Case Management - Incident inspection. LPA met with Center Director Alex Link and informed Director of the purpose for todays inspection. LPA conducted a walk through of the Child Care Center(CCC) with the Director. LPA observed 15 toddlers along with 112 preschoolers and 15 staff providing care and supervision. On 4/1/2025, CCC Director contacted Community Care Licensing (CCL) to self-report the following unusual incident : The class was playing games "freeze dance" and "run and freeze". During the games C1 and peer ran into one another. C1 fell back and hit their head on the floor. C1 immediately started crying and screaming. S1 ran over to comfort C1. S1 brought over Ice however C1 refused Iced. Center staff contacted Parent. The parent took C1 to the ER. It was confirmed at the ER, C1 received a concussion. Today LPA verified the unusual incident written report (LIC624) was emailed to CCL on 4/1/2025. Also LPA verified, the hospital C1 was taken to is Community Memorial Hospital. Continued on LIC809-C Today LPA conducted staff interviews and reviewed children and staff files. LPA requested a children's roster and staff roster. The review of this incident is still on going by CCL. Exit interview Conducted and report was read to Director Alex Link. Notice of site visit was provided and must remain posted in a prominent publicly accessible area of the center .
