On 5/12/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of Initiating a Case Management - Incident review. LPA met with Center Director Alex Link and informed Director of the purpose for todays inspection. LPA conducted a walk through of the Child Care Center(CCC) with the Director. LPA observed 51 children and 9 staff providing care and supervision. On 4/25/2025, CCC Director contacted Community Care Licensing (CCL) to self-report the following unusual incident : During playtime activity, C1 was outside in the preschool play yard on a play structure. C1 climbed the side of the play structure where there is a bridge. C1 jumped off the bridge- railing and landed on right foot(see LIC812). C1 began to cry. S1 comforted C1 and applied ice on to C1's foot. S1 took C1 to the office. While C1 was at the office Director observed swelling on C1's right foot. Staff contacted the parent of C1. Staff advised parent, to take C1 to urgent care. LPA verified the unusual incident written report (LIC624) was emailed to CCL on 4/25/2025. Also the hospital C1 was taken to is Kaiser Urgent Care. Continued on LIC809-C Today LPA interviewed director and reviewed file of C1. Also LPA requested and obtained a children's roster and staff roster. Exit interview Conducted and report was read to Director Alex Link. Notice of site visit was provided and must remain posted in a prominent publicly accessible area of the center .
