What the inspector wrote

On 5/23/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of Initiating and completing a Case Management - Incident review. LPA met with Center Director Alex Link and informed Director of the purpose for todays inspection. LPA conducted a walk through of the Child Care Center(CCC) with the Director. LPA observed 44 children and 9 staff providing care and supervision. On 4/25/2025, CCC Director contacted Community Care Licensing (CCL) to self-report the following unusual incident : During playtime activity, C1 was outside in the preschool play yard on a play structure. C1 climbed the side of the play structure where there is a bridge. C1 jumped off the bridge- railing and landed on right foot(see LIC812). C1 began to cry. S1 comforted C1 and applied ice on to C1's foot. S1 took C1 to the office. While C1 was at the office, Director observed swelling on C1's right foot. Staff contacted the parent of C1. Staff advised parent, to take C1 to urgent care. LPA verified the unusual incident written report (LIC624) was emailed to CCL on 4/25/2025, and was received in the appropriate time frame as outlined in Title 22 CCR. Continued on LIC809-C During the review of the incident, LPA conducted Director interview. Director stated, the hospital C1 was taken to is Kaiser Urgent Care. Also LPA interviewed S1 and according to staff#2, C1 was playing on a structure on 4/25/2025 in the preschool yard ("100 play yard")and when C1 jumped of the bridge, C1 injured their foot. S1 and Director confirmed parent was notified once the physical health of the child had change. Also during the review of the incident, LPA conducted a children's file review and LPA reviewed a medical report pertaining to C1's injury(see LIC812). Today CCC will receive one technical assistance. Exit interview Conducted and report was read to Director Alex Link. Notice of site visit was provided and must remain posted in a prominent publicly accessible area of the center .
