What the inspector wrote

On 04/13/2025, at 9:30 AM, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Annual/Random inspection of the above-mentioned Child Care Center (CCC). LPA met with Lindsay Wozniak, Director of the CCC, and explained the purpose of the inspection. LPA in the company of the Director, toured the interior and exterior of the CCC. The CCC operates from 7:30 AM to 5:30 PM, Monday through Friday. The CCC is a combined program with an infant license and utilizes seven (7) classrooms (Monkey and Panda Room for 2 year olds, Tiger and Duckies Room for 2.5 to 3 year olds, Owls and Geckos Room for 4 year olds, and Lions Room for 4.5 to 5 year olds). LPA observed eight-six (86) children during the inspection, along with nineteen (19) teachers (cleared and associated) providing care and supervision. LPA observed required licensing forms posted near the entry door of each classroom as well as the Director’s office. LPA observed age-appropriate toys and furnishings throughout the CCC. Snacks and lunch are provided by the parents of children in care. The CCC has additional food and beverages should the need arise. LPA observed a menu posted on a fridge in the storage area of the CCC. LPA observed carbon monoxide detectors in the napping rooms of the CCC. A carbon monoxide detector was tested in the Monkey Room at 10:12 AM and found to be operable. Children in care have access to available drinking water both inside the classroom and outside in the play area. Continued on 809-C Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the Licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Medications, sharps, and cleaning supplies were observed to be in areas of the CCC that are accessible to staff and inaccessible to children in care. The restrooms in the CCC are in clean and sanitary operating condition and changing tables are within arm’s reach of sinks. The CCC uses the BrightWheel application for Sign In/Out purposes. LPA verified the entries from the authorized parents/guardians. The CCC’s outdoor play area is fully enclosed, and the entry/exit points are secured. LPA observed age-appropriate toys/play equipment in the play area which are in suitable condition. LPA observed play structures in the playground area have adequate cushioning materials to absorb falls and an abundance of shade provided by shade sails. LPA notes no bodies of water are on site. LPA observed waste receptacles with tight fitting covers throughout the CCC. Staff and children’s records were reviewed. The children’s records were complete and found to contain emergency contact information, immunization records, and other required licensing forms. Staff records were reviewed. At 11:45 AM, LPA’s records review found that Mandated Reporter training is not current for S1, S2, S3, S4, and S5. Additionally, Immunization records were not current for S3, S6, S7, and S8. The remainder of the staff’s records were current with employee rights and CPR/First Aid training. LPA reviewed the CCC’s emergency/disaster log which notes the most recent drill was conducted on 02/11/2026. Continued on 809-C LPA notes there is a security guard with a firearm at the entry of the CCC. Director informed the LPA that the CCC does not have a waiver available for review for regulation 101238(g)(2). Director informed LPA that the CCC will submit a waiver request for regulation 101238(g)(2). This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Continued on 809-C Type B deficiencies are being cited based on LPA's records review pursuant to Title 22 of the CA Code of Regulations (refer to LIC 809-D). Licensee was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Lindsay Wozniak.
