on 9/7/23 and 9/22/23. Based on LPAs observation, record reviews and interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. The CCC will not receive any additional Type A or B Deficiencies for the same incidents which was addressed by CCLD on 9/7/23, and 9/22/23. Further, the CCC submitted a Plan of Correction for the Deficiencies which was accepted by CCLD. No further plans of correction are warranted. A closing interview was conducted with Director. Director was provided and advised of their right to appeal (LIC 9058). A copy of this report was reviewed and provided to Director The Notice of Site Visit (LIC 9213) was also provided to the Director as required by H&S Code Section 1596.817. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may apply. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
