What the inspector wrote

On February 6, 2026, at 1:36PM , Licensing Program Analyst (LPA) Cynthia Alvarez made an unannounced visit for the purpose of conducting a Case Management - Incident inspection. LPA met with Director Amanda Izaguirre to discuss an incident that was self-reported to Community Care Licensing Division (CCLD) office by telephone call on January 22, 2026. LPA toured the classroom and the areas where the incident occurred and interviewed the Director and the staff involved in the incident. During the inspection, there were 66 children present in the supervision and care of 9 staff. On January 22, 2026, at 9:50AM children and staff were in classroom 3 , there was 3 teachers supervising a total of 20 children. There were enough teachers in the classroom to ensure they were within ratio. During this time C1 was able to open the door and run out to the attached next-door classroom 2. When C1 entered classroom 2 there were 3 teachers present, not supervising children, they were on break or preparing lesson plans. All 3 staff ran up to attend to C1; however, C1 was able to run into the teacher’s office and lock it. Staff member ran to obtain the key and returned within a few seconds, they were unlocked the door; however, C1 was pushing against the door so that staff members could not open it. Eventually after a few seconds more, staff members were able to open the door. C1 safe and was laughing. C1 parents were notified and they were understanding and stated this is typical behavior for C1. C1 returned to the center the next day after the incident. To prevent similar incidents, the center is utilizing a positive behavior support plan, in which includes meeting with parents, teachers and upper management to ensure the child’s needs are being met. Additionally, the center is reviewing doors in the classroom and center to verify which ones will need to remain closed and locked, they have decided to keep the door to the teacher’s office locked to prevent children from entering. Given the center took all appropriate actions, such as reporting to licensing in a timely manner and submitting a written report regarding the incident, no citations will be given regarding this incident. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Appeal Rights were provided to Site Supervisor Exit interview conducted and report was reviewed with Director. Amanda Izaguirre.
