What the inspector wrote

On 4/2/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced visit for the purpose of completing a Case Management - Incident inspection. LPA met with Eva Gonzalez Early Childhood Education Assistant Manager(ECEA) and informed ECEA of the purpose for todays inspection. LPA conducted a walk through of the Child Care Center with the Center ECEA. LPA observed at the time of the walk-through 35 children being supervised by 6 staff. On 10/18/2025, Facility representative contacted Community Care Licensing (CCL) to self-report the following unusual incident : C1 was out on playground running around and somehow tripped and fell. Landed/ braced themselves with the palm (right hand). C1 was favoring right hand and saying that it was hurting. Facility representative Contacted(telephone) parent(p1) to pick C1 up and to get C1 checked out by a doctor. Babysitter picked up at regular time. Site Supervisor called Parent in the afternoon and encouraged Parent to take C1 to urgent care. C1 returned to school on 10/17/24 with doctors note stating no restrictions. Parent said that C1 'arm' was barely broken. Continued On LIC809-C The additional incident under review by CCL, was self reported to the department on 10/14/2024. The reporting party stated: C1 was laying on the ground on the playground and another child came up and grabbed C1 by the right wrist to pull C1 up to play. C1 cried a little bit after and pointing to hand. C1 was picked up early by babysitter. No doctor visit was needed. Today LPA verified the 10/16/2024 and 10/14/2024 , had a unusual incident written report (LIC624) submitted to CCL on 10/21/2024. Today LPA conducted staff interviews for both aforementioned unusual incidents. Also LPA completed children's file reviews. LPA found the 10/16/2024 incident report, was not signed by C1's authorized representative. Today the facility will receive a Technical Violations. Exit interview Conducted and report was read to Facility Representative, Eva Gonzalez(ECEA). Notice of site visit was provided and must remain posted in a prominent publicly accessible area of the center .
