What the inspector wrote

On 7/30/25 at time, Licensing Program Analysts (LPAs) Veronica Diaz and Brian Fung made an unannounced inspection for the purpose of conducting a Case Management inspection. LPAs met with director Debbie Blumenthal to discuss an incident that was self reported to Community Care Licensing Division (CCLD) office by phone on 5 separate occasions dated 01/08/25, 01/10/25, 02/27/25, 03/21/25, and 05/02/25. Together with the director and LPAs toured the facility inside and outside. At the time of inspection there were 156 preschool children and 24 Toddler and 39 staff were present. LPAs observed center and interviewed director about every incident reported. Director stated children were not in-distressed and has no prolonged effects related to the incident . The parents were notified also stated parents were grateful that the center contacted them and took action. The children from all 5 the incidents continued to be enrolled in the facility and parents have not expressed terminating services. LPAs and Director discussed active supervision. Director reported CCC has conducted investigation and made proper updates to ensure this incident does not occur again. The circumstances were investigated by the administrator to determine what factors contributed to the incident and steps to prevent it from happening again are in place. The expectations regarding safety and supervision are and will continue to be strongly communicated to our staff through training's and meetings and possibly termination if requirements are not met. Staff has received a written documentation of corrections, and it will be recorded in her personnel files. 2 out of the 5 staff involved in the incidents are no longer working at the center and was stated they voluntary left. CONT 809-C During today\342\200\231s inspection technical violation was provided regarding active care and supervision. Exit interview and review of report was conducted with Director Debbie Blumenthal Notice of Site visit was provided and must remain posted for the next 30 days.
