On Septemnber 18, 2025 at 10:45 AM, Licensing Program Analysts (LPAs) Brian Fung and Veronica Diaz made an unannounced inspection for the purpose of conducting a Case Management inspection. LPAs met with Director Debbie Blumenthal and Assistant Director Keri Loventhal to discuss an incident that was self-reported to Community Care Licensing Division (CCLD) office by phone on 9/4/2025. LPAs toured the area where the incident occurred and interviewed Director and Assistant Directors. Both director and assistant director discussed with the classroom teachers in which the incident happened that supervision is a priority and to refer back to the training from February and March 2025. Given the incident was observed by staff who took appropriate action by reporting to the director; The directors reporting to CCLD. Because this incident is reoccurring a type B deficiency is being issued as a result of this incident. LPAs request the director come to Santa Barbara Regional Office for a compliance office visit. LPAs investigated this incident and based on interview with director and assistant director, there was sufficient evidence in lack of supervision to support that resulted in C1 being left unsupervised by the toddler gate. Therefore, the following deficiencies will be sited today 101229(a)(1) CONT 809-C Following the incident C1 continues to be enrolled in the CCC. During today\342\200\231s inspection, a type B violation was issued regarding active care and supervision. Exit interview and review of report was conducted with Director Debbie Blumenthal and Keri Loventhal. Notice of site visit and appeal rights were provided and must remain posted for the next 30 days.
