What the inspector wrote

On 03/10/2026, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management Inspection of the above-mentioned Child Care Center (CCC) as a follow up to a self-reported Unusual Incident Report (UIR) received by the Department on 01/21/26. Circumstances of the UIR involved a child (C1) being left unattended while transitioning to a soccer activity. LPA met with Debbie Blumenthal, Director of the CCC, and explained the purpose of the inspection. LPA notes one hundred and thirty-nine (139) children are on site during the inspection along with twenty-eight (28) staff. LPA discussed the incident with the Director. Director explained that the 2 year-old class (approximately six children) was being transitioned by A1, a third-party enrichment program employee, to the CCC\342\200\231s auditorium for a soccer activity. Director confirmed C1 was left outside of the auditorium lobby in the front lawn area (Warren Lawn) of the CCC, near Classroom N. S1 (Teacher in Classroom N) noticed C1 by the window and escorted C1 into the auditorium lobby to join the class. Director states C1 was left unattended for approximately 1 \342\200\223 2 minutes. LPA verified A1 is a fully qualified teacher. Director immediately informed C1\342\200\231s parents of the incident and informed CCLD later the same day. LPA notes C1 was not injured or harmed during the incident and is still attending the CCC. Additionally, LPA was informed that the CCC terminated employment with the third-party enrichment program. Continued on 809-C A Type B deficiency and Civil Penalty of $250 for a repeat violation within 12 months is being cited in accordance with Title 22 of the California Code of Regulations and Health and Safety Code. Please refer to LIC 809 D for documentation of deficiency cited: A notice of site visit was given and must remain posted for 30 days as required by H&S Code Sec.1596.817. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may apply.

This requirement was not met as evidenced by: Based on records review and interview, the unusual incident occurred on 01/27/2025 and was not reported to CCLD until 01/30/2025. This poses a potential health, safety, or personal rights risk to persons in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time...This requirement is not met as evidence by: Based on records review and interview, the licensee did not comply with the section cited above in that C1 was left unattended outside the CCC's auditorium for approximately 1-2 minutes. This poses a potential health, safety or personal rights risk to persons in care. LPA notes this is a repeat violation within 12 months. CCC assesed a civil penalty of $250 per H&S 1548(2)(A).
