On 03/10/2026, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management - Incident inspection at the above-mentioned Child Care Center (CCC) as a follow up to a self reported Unusual Incident Report (UIR) received by the Department on 01/30/2025. LPA met with Director, Debbie Blumenthal and explained the purpose of the inspection. LPA notes one hundred and thirty-nine (139) children are on site during the inspection along with twenty-eight (28) staff. On 01/30/2025, the CCC reported to CCLD that C1, C2, and C3 were engaged in inappropriate behavior in the play yard. During the inspection LPA interviewed the Director regarding the incident that occurred. Interviews revealed that 3 staff (S1, S2, and S3) were present when the incident occurred and were able to intervene immediately. Interviews also revealed this is an isolated incident and that the CCC worked with the families involved to ensure further instances of inappropriate behaviors do not occur. Interviews also revealed parents of the children involved were informed of the incident and that the children continued to be enrolled at the CCC until recently moving onto TK at another facility. LPA determined the CCC had adequate supervision at the time of the incident and informed the parents of children involved within 24 hours. However, the incident occurred on 01/27/2025 and was not reported to CCLD until 01/30/2025. Based on records review and interview, a Type B deficiency is being cited in accordance with Title 22 of the California Code of Regulations.
