101212 Reporting Requirements(d)Upon the occurrence, during the operation of the child care center…a report shall be made to the Department…within the Department's next working day…following the occurrence of such event.(1) Events reported shall include...(C) Any unusual incident…that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by: Based on records review and interview, the unusual incident occurred on 01/27/2025 and was not reported to CCLD until 01/30/2025. This poses a potential health, safety, or personal rights risk to persons in care.

101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...This requirement is not met as evidence by: Based on records review and interview, the licensee did not comply with the section cited above in that C1 was left unattended outside the CCC's auditorium for approximately 1-2 minutes. This poses a potential health, safety or personal rights risk to persons in care. LPA notes this is a repeat violation within 12 months. CCC assesed a civil penalty of $250 per H&S 1548(2)(A).
