What the inspector wrote

On 12/17/2025, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management Inspection of the above-mentioned Child Care Center (CCC) to address an incident report received by CCLD on 08/15/2025. Specifically, Director Shannon Meyerson reported that on 08/12/2025, C1 pushed C2 causing an injury that required medical treatment. LPA met with Shannon Meyerson, Director of the CCC, and explained the nature of the inspection. Director provided LPA with an account of what transpired. Director informed LPA that while C1 and C2 were in the play yard, C1 pushed C2, causing C2 to fall. C2 sustained a cut to his forehead which required medical attention. Director informed C2’s parents immediately after the incident occurred. Director informed LPA that C2’s parents chose not to seek medical attention for the injury. Director followed up with the parents regarding the incident the following day (08/13/2025) and the child returned to the CCC. During LPA interviews, the Director acknowledged that the incident should have been reported to CCLD within 24 hours/the next working day. Director could not recall the specific reason that the incident was not reported to CCLD within the required time frame. A Type B Deficiency is being cited based on LPA’s interviews pursuant to Title 22 of the CA Code of Regulations (refer to LIC 809-D). Director was provided a copy of their Appeal Rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days or a civil penalty of $100 may apply.
