What the inspector wrote

On April 23, 2026 Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management - Incident inspection at the above-mentioned Child Care Center (CCC). LPA met with Teacher Samantha Ramirez and informed them the purpose of the inspection is to follow up on the self reported incident. At the time of the inspection 29 children and 4 staff were present. On 4/3/2026, the CCC self reported an incident where a child (Child 1 (C1)) fell on the monkey bars and injured their bottom. Staff checked on C1 and C1 then returned to playing . After being pick up parent reported taking the C1 to the emergency room where it was concluded there was only bruising. LPA in the company of the teacher toured the area where the incident occurred. LPA observed the play structure to be age appropriate for the children in care. LPA observed the footing of the play structure to be rubber cushioning. Teacher Samantha Ramirez stated the observed the incident occur and clarified C1 fell on the metal part of the climbing area of the play structure. The incident was determined to be risk at play. The CCC took the appropriate steps in ensuring C1 was fine after the incident, notifying parents, and reporting the incident to CCL, no deficiencies are being cited as a result of this incident. Exit interview was conducted with Samantha Ramirez and notice of site visit was given.
