Incident investigation·February 12, 2026

On February 12, 2026 Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management - Incident inspection at the above mentioned Child Care Center (CCC). LPA met with Director Autumn McGuire and informed them the purpose of the inspection. At the time of the inspection 5 infants were present. On 1/30/26, the CCC self reported an incident where Child 1 (C1) was playing on an indoor play structure, slipped and fell. C1 was taken to urgent care where it was found C1 had sustained 2 fractures in their arm. LPA in the company of the director toured the area of the incident. LPA interviewed director regarding the incident as well. LPA requested a copy of the play structure's user manual/instructions and contact information of parents of C1. Due to insufficient information at this this time the above mentioned incident requires further follow up. Report was reviewed with Director Autumn McGuire. Notice of site visit was given.
