What the inspector wrote

On April 24, 2026 Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management inspection at the above mentioned Child Care Center (CCC). LPA met with Teacher Margarita Galeas and informed them the purpose of the inspection is to conclude an incident follow up. At the time of the inspection 5 children were present. On 1/30/26, the CCC self reported an incident where Child 1 (C1) was playing on an indoor play structure, slipped and fell. C1 was taken to urgent care where it was found C1 had sustained 2 fractures in their arm. On 2/12/2026 LPA initiated follow up regarding the incident. LPA observed the play structure was in good condition and that the there was padding surrounding the play equipment. Further, the director provided the play structure description which stated that the equipment's intended use is for children ages 1-3 years old. Based on the information obtained the incident is deemed risk at play. No deficiencies are being issued as a result of the incident. Report was reviewed with Teacher Margarita Galeas and notice of site visit was given.
