Inspector’s narrative

What the inspector wrote

On 10/10/2025, Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced inspection at the above-mentioned Child Care Center (CCC) to conclude an incident investigation. LPA met with Director Jessica Derby and informed them of the purpose of the inspection. At the time of the inspection no children were present. On February 11, 2025, the CCC self reported an incident involving Staff 2 (S2) and children C1 and C2. While walking to the on-site caf\303\251, the children approached a dog to pet it. The dog became excited and snipped at C1\342\200\231s face. During the investigation, staff were interviewed, Parent of C1 was interviewed and CCC documents were reviewed. Interview with the Director revealed immediately after the incident, Director informed all staff that they are not to approach dogs while caring for children. Further, the Director updated the Expectations & Accountability portion of the staff handbook to specifically disallow staff and children from approaching or petting dogs. Additionally, it was updated to specifically disallow staff from taking children to the caf\303\251 for personal reasons. Interview with Staff 2 revealed they generally discourage children from petting dogs. However, during this incident the 2 children with Staff 2 were petting an unattended dog. Further, Staff 2 stated they immediately took children inside the caf\303\251 to treat C1\342\200\231s injury. Staff 2 also notified Parent of C1 who then took C1 to the emergency room. Interview with Parent of C1 revealed they are very happy with the center and what occurred was an unfortunate incident. CONTINUED PAGE 2 Further, documents reviewed revealed the campus the CCC is on, has a process for allowing service animal on the campus and that the owner of the dog went against campus policy in leaving the dog unattended. The incident report provided to CCL stated the dog was immediately removed from campus and was no longer allowed to return. Further the Director notified CCL that S1 is no longer employed at the facility. Based on the information obtained, the CCC did not ensure C1\342\200\231s safety, however responded appropriately to the incident. A Technical Violation is being issued as a result of the incident. No deficiencies were issued due to the incident. Report was reviewed with Director Jessica Derby. Notice of Site Visit was given.
