Inspector’s narrative

What the inspector wrote

On 4/04/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of completing/closing a Case Management - Incident inspection. LPA met with Center Director Leon Paul and informed Director of the purpose for todays inspection. LPA conducted a walk through of the Child Care Center with the Center Director. LPA observed at the time of the walk-through 44 children being supervised by 5 staff. On 1/16/2025, Program Specialist contacted Community Care Licensing (CCL) to self-report the following unusual incident : On approximately 8/15/2024 C1 was playing outside in the play ground with the rest of the children in the "two's" classroom. S1 and S2 were providing care and supervision to the children. According to S1 during transitioning, from outside to inside, C1 was brought into the Two's classroom, and was checked off on the "name to face" document. S1 stated, how S1 was informed by S3, how C1 was still out side in the play yard. It is still unclear who returned C1 back inside the two's classroom. The additional incident under review by CCL, was self reported to the department on 8/01/2023. The reporting party stated: A parent approached Assistant Director (AD) and stated teachers were giving C2 gummy bears with melatonin. Parent is claiming when picking up C2 at the center, parent over heard, teachers were giving out gummy bears as a reward system for potty training. Continued On LIC809-C Allegedly these gummy bears had Melatonin inside of them and Teachers would place the gummy bears in a baggy and send the gummy bears home with the children(See LIC812). Today LPA verified the 8/14/2024 unusual incident written report (LIC624) was emailed to CCL on 1/23/2025. Due to the length of time (years), the date for the submission of the 8/01/2023 LIC624 is unclear(see LIC812). LPA conducted staff interviews for both unusual incidents. LPA requested and received a children's roster and staff roster. Also LPA received CSR, child supervision record document. Additionally CCC conducted their own investigation into both incidents(see LIC812). In regards to the 8/01/2023 incident, the staff interviews revealed how no one witnessed or could collaborate the claim, staff were giving melatonin gummy bears to children. LPA interviewed former director and showed LPA the alleged gummy bears in question(see LIC812). Former director stated the staff never gave melatonin gummy bears to children in care. In regards to the 8/14/2024 incident, due to inconsistent statements by CCC staff, LPA could not determine a exact narrative of which staff made the mistake when C1 was left unattended in the play yard. The staff and director interviews revealed, C1 was left unattended for at least one minute. Today CCC will receive one technical violation and one type B violation. Exit interview Conducted and report was read to Director Leon Paul. Notice of site visit was provided and must remain posted in a prominent publicly accessible area of the center .
