What the inspector wrote

On July 25 2025, Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management - Incident inspection at the above-mentioned Child Care Center (CCC). LPA met with Director Leone Paul and informed them the purpose of the inspection is to address incidents regarding children being left unattended. At the time of the inspection 43 children were present. On 1/16/2025, the CCC self reported an incident that occurred in August of 2024, where is a child was left unattended. 1 Type B deficiency was issued due to this incident. On 3/25/25, the CCC self reported an incident where a child was left inside the classroom. According to the report staff brought some children inside for diaper changes. When going back to the playground, 1 child was left behind in classroom. On 6/6/2025 the CCC self reported and incident where a parent alerted a staff that a child was left inside the classroom, as staff were taking children to the play ground. According to the report the child was left without supervision for about 10 seconds. On 7/22/2025, the CCC self reported an incident where during a transition to the play ground, a child was left inside the classroom. According to the report the child was left unsupervised 1-2 minutes. Due to the pattern of the lack of supervision, 1 Type A deficiency is being issued on the attached LIC 809D and a Civil Penalty is being assessed. CONTINUED PAGE 2 Upon receipt, provide copies of this licensing report to each parent/guardian of enrolled children and to parents/guardians of newly enrolled children during the next 12 months. Acknowledgement of Receipt LIC 9224 form shall be used for this purpose. LIC 9224 after completed shall be maintained in each child's file. (LIC 9224 was provided to director). Report was reviewed with Director Leone Paul. Notice of site visit was given and appeal rights were given.
