Inspector’s narrative

What the inspector wrote

On January 29,2026, Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management inspection at the the above mentioned Child Care Center (CCC). LPA met with Director Leone Paul and informed them the purpose of the inspection is to follow up on a self reported incident. At the time of the inspection 28 children were present. On 12/10/25 the CCC self reported an incident where Child 1 (C1) was missing a milk bottle and that Child 2 (C2)had taken the bottle home with them. LPA notes per the report, C1's missing bottle was found empty by C2's parent. LPA reviewed statements made by staff regarding the incident and reviewed the CCC's bottle feeding log. LPA observed C2 arrived with 4 breast milk bottles. LPA notes C2 was fed at 8:10AM, 11:01AM, and 1:20PM. C1 arrived to the facility with 3 breast milk bottles. LPA notes C1 was fed at 10:28AM and 1:07PM. Staff 1 (S1) stated at 9:30AM, C2 had 4 bottles remaining and C1 had 2 bottles remaining. Additionally, according to statement made by Staff 2 (S2), at 1:30PM C1 had no remaining bottles and C2 had 2 bottles remaining. Staff 3 (S3) stated the only child they fed in the Infant A , was C2 which is consistent with the feeding time of 8:10AM. Based on the information obtained, it is more likely than not that C2 was fed C1's breast milk in error. 1 Type A deficiency is being issued on the attached LIC 809D. LPA notes since the incident, the facility has set up additional steps to prevent similar incidents from reoccurring. Additionally the CCC responded appropriately in notifying Community Care Licensing and notifying parents. Report was reviewed with Director Leone Paul. Notice of site visit and appeal rights were given.
