What Happened
Child care center incident investigation following unsupervised child exit from classroom. Child exited due to broken gate and staff inattention but remained within facility.
Observations
- Child #1 exited classroom unsupervised and walked down hallway near bathroom
- Broken child gate allowed unsupervised exit from classroom
- Two classroom staff did not observe child exit
