What the inspector wrote

On 10/30/25, Licensing Program Analysts (LPAs) Veronica Diaz and Cynthia Alvarez conducted an unannounced Required-3 Year inspection. LPAs met with office staff/facility representative Karleen Cole and advised them of the purpose of the inspection. facility representative stated they have not had a director since last year the prior director on file retired and 3 lead teachers are currently assisting as co-directors. facility representative provided LPAs a tour of the facility inside and out due to co-directors being in classroom for ratio purposes. There were 5 infant in care at the time of the inspection along with 2 staff. Center operates Monday - Friday from 7:30AM-5:30PM. LPAs observed required license at the entrance of the facility office. LPAs did not observe PUB 269 (Child passenger restraint system poster), PuB 393 (Notification of Parents Rights), and LIC 610 (Emergency Disaster Plan) at the entrance of the center, however postings are posted in childcare room. Facility representative stated public has access. LPAs reminded office staff of the required document to be posted in a prominent, publicly accessible area in the facility. LPAs observed parents/guardian sign-in/sign-out clipboards at the entrance of the facility. In the infant classroom LPAs observed infant with asleep sack LPAs reminded infant teacher and facility representative that sleep sacks are not allowed with out a waiver or exception. Each of the classrooms have age-appropriate toys and furniture readily accessible for infants in care. LPAs observed changing table at arms reach of sink. LPAs did not observe any hazards/toxins items accessible to infants. First Aid kit was observed in the infant room. The facility communicates with parents/guardians by email, phone, and app called Transparent Classroom. Facility also sends out monthly newsletters with menus. Facility does provide 2 snacks and lunch for purchase or children will bring there snacks and lunch from home. LPAs observed menus posted throughout the facility. LPAs observed refrigerator in the infants classroom with labeled containers for each infants milk and food storage. Continued on 809-C The outdoor playground is shaded and has enough amount of space for infants to play. LPAs observed the playground has age-appropriate toys and structures available for infant to use. A sampling of 5 infant and 1 staff records were reviewed. LPAs observed children's and staff files to be complete, however LIC9227 Individual Infant Sleeping Plan was missing in infants record and for staff record LIC503 Health Screening is missing. Staff mandated reporter certification are all current. LPA verified SB792 Child Care Adult Immunization and Tuberculosis requirements. For additional information regarding the inspection and its tools and methods, please visit the Program website at www.cdss.ca.gov/inforesources/community-care-licensing/inspection-process . facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Continued on 809-C LPAs discussed the safe sleep regulations with licensee facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. No deficiencies 3 Technical Violations and 2 Technical Advisories where issued at today's visit A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted, appeal rights were given, and report was reviewed with co-directors Jenny Lang and Renee Warner.
