What the inspector wrote

On November 26, 2025 at 1:30 PM, Licensing Program Analyst (LPA) Brian Fung conducted an unannounced Annual inspection. LPA met with licensee Berenice Rivera and advised the purpose of the inspection. Licensee provided LPA a tour of the home inside and out. There were six children in care at the time of the inspection and one adults in the home. LPA observed required Licensing required documents posted prominently at the entrance. Fire and earthquake drills are being documented every six months. Last drill was conducted on 6/17/2025. Fire extinguisher 2A10BC mounted on the wall inside the school room was last purchased on 9/10/2025. Fire and carbon monoxide detectors were tested at 1:55 PM and found to be in working condition. Children in care have access to 2 rooms for play and sleep, one restroom, kitchen, and front yard. Fireplace is screen and covered with furniture's to prevent children from access. Kitchen cabinets were closed and knives are being kept in a cabinet above counters to keep out of reach from children. Cabinets do not contain any materials that would pose a danger or risk to children. Children have access to toys that are age-appropriate inside and outside of the home. Front yard play area is enclosed and has plenty of play structures and activities. There are no guns and/ or ammunition located on the premise. There are no bodies of water present at the time of inspection. During the tour, LPA did not observe any hazards/toxins items accessible to children in care. Continued on 809-C LPA reviewed six out of six children files. Four out of six children files are missing immunization records. One out of six chidlren files is missing individual infant sleep plan. Licensee CPR/first aid is valid through 10/20/2026. Licensee was reminded a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter certification AB1207 every two years at www.mandatedreporterca.com Licensee\342\200\231s mandated reporter certification is missing. Licensee discussed that they have not renewed their mandated reporter training. A Type B deficiency was cited for today for missing mandated reporter training. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee discussed that the 15 minute checks were not being conducted. A type B deficiency was cited for missing 15 minute infant sleep checks and individual infant sleep plan. Continued on 809-C Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the LICENSEE Berenice Rivera, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. There were 3 type B deficiencies cited on today's visit. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal rights were given. Exit interview conducted and report was reviewed with the licensee Berenice Rivera.
