What the inspector wrote
On 11/04/2025, at 12:35 PM PST, Licensing Program Analysts (LPAs) Seena Parsapour & Shane Loftus conducted an unannounced Annual/Random Inspection of the above-mentioned Child Care Center (CCC). LPAs met with the Site supervisor/facility representative, Mayra Brady, and explained the purpose of the inspection. LPAs, in the company of facility representative, toured the interior and exterior of the CCC. The CCC utilizes four (4) classrooms along with outdoor play areas designated for specific age groups. The CCC operates from 7:00 AM to 6:00 PM, Monday through Friday. At the time of the inspection, LPAs observed forty eight (48) children in care, along with nine (9) staff members providing care and supervision. LPAs note that this CCC has a toddler component.
LPAs observed required licensing documents posted near the CCC\342\200\231s entry door. The CCC uses paper Sign In/Out log documents. LPAs note the documents contain the children\342\200\231s arrival and departure times as well as entries of the authorized representatives/parents. The CCC is clean and orderly. LPAs observed age-appropriate toys and furnishings throughout the CCC. LPAs observed carbon monoxide detectors in each of the four classrooms, and the carbon monoxide detector in the Constructive Triangles classroom was tested at 12:56 PM and found to be operational. LPAs note that each of the four classrooms contain designated bathrooms for the use of children in care. The CCC's bathrooms were found to be safe, sanitary, and in operating condition. In each of the four classrooms, LPAs observed sharps, medications, disinfectants and cleaning solutions, and other hazardous items stored in areas inaccessible to children in care.

Children bring snacks and lunches from home, and the CCC provides snacks as needed. LPAs observed a menu posted prominently in the kitchen area of the CCC. The CCC has an operational refrigerator in the kitchen area. Filtered water is provided for children in care inside of the facility and outside in the play yard.
CCCs constructed before January 1, 2010, are required to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPAs verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
LPAs observed the outdoor play areas to be fully enclosed. There are four separate outdoor play areas, partitioned off by way of fencing and latched gates. LPAs note the entry/exit points are secured. LPAs observed age-appropriate toys/play equipment in the play areas. The toys/play equipment are free of sharp, loose or pointed objects and there is adequate cushioning to absorb falls. LPAs observed multiple locked sheds in the outdoor play areas that contain center supplies. There is an abundance of shade in the play yards created by shade sails and trees. The footing is varied in the outdoor play areas between rubber matting, concrete, grass, and sand. No bodies of water were observed on site.
Staff and children\342\200\231s records were reviewed. The children\342\200\231s records were current and complete with emergency contact information, immunization records, and other required licensing forms. Staff records were found to be current & complete with current Mandated Reporter training, teaching qualifications, and other required licensing forms. LPAs reviewed the facility's fire/emergency drill log and notes the last drill was conducted on 10/06/2025. Facility representative informed LPAs no firearms/weapons or ammunition are stored on the premises.

LPAs discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPAs also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
